Root Restore Health, LLC is based in Florida and provides virtual functional and integrative health education and wellness consulting to adults age 18 and older only. Prospective clients and service area
International inquiries are welcome. Every prospective client is reviewed individually before acceptance, with particular attention to inquiries from outside the United States. Services are subject to applicable laws in the prospective client's location.
Submitting an inquiry or assessment does not establish a client relationship, book an appointment, or guarantee acceptance.
Information we may collect
The website's public inquiry forms collect only your name, email address, and an optional general message.
Please do not include symptoms, diagnoses, medications, laboratory results, or other private health information in this form. Do not use a website inquiry form, ordinary email, or a social-media message to submit health-intake information, medical records, passwords, or portal credentials.
If you choose to complete the Practice Better assessment, information may include health history, symptoms, nutrition and fasting information, current prescription medications and supplements, laboratory documents you choose to provide, required acknowledgments, a signature date, printed name, and electronic signature.
Use the separate Practice Better assessment for the health-intake information requested there. Public assessment submissions remain prospective-client submissions pending review.
How information is used
Information may be used to respond to inquiries, review prospective-client submissions, arrange consultations, provide requested educational wellness services, maintain appropriate business records, communicate about those services, and operate or protect the website and business.
Detailed intake responses are not used for public marketing without separate permission.
Services involved
Practice Better is used for the public health-intake assessment and related records. The prepared website is intended to be hosted by Netlify. The website inquiry forms are configured to use FormSubmit to transmit the name, email address, and optional general message entered in those forms to the Root Restore Health business email. Google Fonts supplies website fonts, and the website includes links to Instagram.
When you use or open a third-party service, that provider may receive the information you submit and technical information ordinarily associated with the connection. Each provider handles information under its own terms and privacy notice.
How information may be shared or accessed
Within Root Restore Health, the owner is the only person authorized to review client assessments and health information. Practice Better and other service providers necessary for the applicable workflow may process information under agreements that apply to their services. Information may also be disclosed with your authorization, to protect rights or safety, or when required by law.
This notice does not make a determination about Root Restore Health's HIPAA status or claim that a particular business associate agreement, data-processing agreement, vendor security control, or other arrangement has been verified.
How information is protected
Root Restore Health seeks to limit access to information and uses third-party services for the functions described in this notice. No electronic transmission, storage system, or service can guarantee absolute security. Use the Practice Better assessment rather than ordinary email or a website inquiry form when providing the intake information requested there.
Retention and privacy requests
Prospective-client records are reviewed individually. A prospective-client record may be deleted after a reasonable review period or retained temporarily when justified by a continuing inquiry or possible future consultation, subject to applicable legal obligations. Root Restore Health does not promise a fixed deletion period until its procedures and the relevant platform capabilities have been verified. Other retention periods may vary based on the type of information, why it was collected, operational needs, and applicable legal or contractual requirements.
You may contact stan@rootrestorehealth.com to ask about information associated with you or to request access, correction, or deletion. Root Restore Health may need to verify your identity and may retain information when required or permitted for legal, operational, security, or recordkeeping purposes.
Your choices
You may choose not to submit optional information and may ask whether another method is available. Some information may be needed to review an inquiry or provide a requested service. You may also ask to stop optional promotional communications.
